Make an Appointment
To make an appointment, please call the main office at (518) 374-3514 between the hours of 8:00 AM and 4:30 PM. Counseling appointments are generally available between 8:00 AM and 8:00 PM.
Please review our Privacy Policy and Client Information
New Clients, please download and complete the Intake Forms to the left. Bring them with you to your first session. CDPHP Clients may choose to complete the optional Wellness Assessment forms also.
